how do i setup a screen to collect email addresses for contest?

I would like to set up a screen that collects customer email addresses on a
laptop pc at the front of a retail store. This screen would advertise various
products to win or advertisments, and have a box to enter an email address
that will be stored in a file transferable to outlook for upcoming
promotions. Any help or even pre-programmed solutions would be great!

Don't if this is too simple for what you're looking for but I did see one of
these a while ago. Basically it was an access form (set to data entry = yes)
with only a "Enter E Mail Address" text box and a Save Button and OnClick
(save button) the form simply refreshed.

I didn't take much notice of it but I would think it was linked to a simply
tbl with one an ID - timedate collector and an e mail address field.
