How do I setup a "form" to send email?

I send a confirmation email containing information to the same person many
times a day. Is there a way to setup a "form" (don't know if this fits the
definition of an Outlook Form) that would allow me to tab through fields
entering data then email it to the receipient with an attachment? For
example:

Customer PO#:
Customer #
My Sales Order#
ETA:

I don't know if this is what you are looking for but a simple way that I do
things like this is create a signature with the fields you want and when you
either send a new e-mail or reply to this person, use this signature, fill in
the information and send it.

I hadn't thought about that. I would like to make something that looked a
little more professional because I may be sending it to more than just this
one customer in the future. Here is where I am now:

I created an Outlook template (.oft) with an embeded Excel form. This works
pretty well and looks great. What I would like to know now is how can I
launch a new email with this template without having to go through picking
the form in Outlook? Desktop Icon? Outlook toolbar button? Hmmmm.

I don't know much about it, but maybe you could try going to to
Tools/Forms/Design a Form... and start with the "Message" one. Instert that
excel form and save it. Then when you compose a new e-mail, go to New/Choose
form... and start with that. -But that doesn't answer filling out the fields
quickly does it? I dunno.

I bet I can create a macro to launch the template, then create a toolbar
button for the macro. I will get lucky and someone will respond quickly, or
I will pick away at the help files until I find it. Or maybe Google will
help. Thanks for you suggestions!

Test sending it to an account that you check using OE or other non-Outlook
client. Published forms may be sent as RTF and are usable only in Outlook.

You can save an oft to the hard drive then copy it to a folder in Outlook
for easy access.
