How do I insert a check box into a header

I have been trying to insert a check box into a header created in Word 2000
that will allow users to toggle a check mark on or off in the box. When I
call up the Form toolbar while editing the header, all the form fill-in
options including check boxes are grayed out. Is there another way to do
this?

You cannot place form fields inside a header (or footer). What you can do is
put the check box in a frame (there is an Insert Frame button on the Forms
toolbar), and drag the frame to position it on top of the header area.
