How do I create a form that allows for anonymous submission?

I need to create a form in Front Page that allows the user to remain
anonymous. On submit, the form information will be sent via email. How do I
fix the form so the email address of the user is not shown?

The email address of the person submitting the form not shown to begin with.
The only way you can get it is to ask them to enter it in a field in the form.

By default, the email is sent from an address on your host server, not the
sender. Don't put a field in the form requiring them to input their email
address.
