Forms stopped working?

I have three forms on my website and all three have stopped working...I have
uninstalled and reinstalled the front page extensions on the server...I have
republished the entire site....I have uninstalled and reinstalled front
page..I have wiped out the form and rebuilt it...still doesn't work.When I
click submit it says this page cannot be found, page may have been removed or
had it's name changed...Here is what is strange..if after publishing in Front
Page I go directly to the web site by clicking the button that says "Click
here to view your published web site" and go to the page with the form the
form works fine....but if I go to the site through IE or have someone else
try the form does not work...The address of the page is

The other day I was in the C drive and wiped out some files...I am wondering
if I did something that is causing this...Please explain in simplest terms
possible how to fix this or work around it....The settings in form properties
are the default save to _private/form_results.csv and I also have them going
to an email address.
The other thing I noticed is that this icon appeared on my desk top
_vti_cnf...that might have been one of the ones I wiped out on c drive...so I
moved it back over there. PLEASE HELP ASAP

Remove the spaces from the filename.
Never use spaces in files or folder names.

First remove ALL spaces (%20) from all file and folder names in your site.

examples:

Newsletter%20Sign%20Up.htm
thank%20you%20newsletter.htm
