Formatting Toolbar: completely gone

I'm using OL 2003.

I don't know why, but my Formatting Toolbar *completely* disappeared
from the composing window, even if the message is HTML formatted.

Right click on toolbar, View > Toolbar and Customize DO NOT show
"Formatting" anymore.

The only thing that show the FT is somewhere is the "New..." button in
Customize / Toolbar window: if I try to create a toolbar with name
"Formatting" it says "A toolbar named 'Formatting' already exists. Type
another name.".

Please... HELP!!!!

BTW: I have also "Form Design" and "Respone" toolbars which I can not
activate.

Word or Outlook as the e-mail editor?

Reset the Toolbar; all Toolbar customizations will be lost and the Toolbar
will reset to what it looked like when you first installed Outlook. To do
this locate outcmd.dat and rename it to outcmd.old when Outlook is closed.
By default you can find it here;
C:\Documents and Settings\%username%\Application Data\Microsoft\Outlook

I'm using Outlook as mail editor.

Do you know what the other two toolbars are?

Response is being used when responding to meeting requests.
Form Design is being used when you are designing/programming customized
Outlook forms.
