Form - remove link to Address Book

I am trying to update a custom form (one I did not create)
and there seems to be a link to a name.

I have a To... (this is where the name appears)
and a CC... which works fine.

Help, I don't know how to fix this. I have recreated the
form and the same thing happens. Where would I look, there
is nothing in VB.

Try running the form, removing the recipents, then
republishing the form. Also check the code to see if
recipients are being added behind the scenes.
