Form not sending e-mail

I have created a form on a page of my site that seems to
do everything including confirm the submission, but it
won't send to the e-mail address I've entered.

I have another form on another page of my site that is
configured exactly the same, though with different
elements and to a different e-mail address), that does
work.

Try using the email address that work in the other form, in this form.

Be certain that the email address that is having problems
is a valid address. You could test it by copying it
(Ctrl+C) and pasting (Ctrl+V) it into your email program
and trying to send a test message.
