Form Drop-down Boxes

I want to create a form with "layered" drop-down boxes. I have 2 main
categories, 15 sub-categories and several categories that fall under the
sub-categories. For example:

Business and Litigation are the main categories, some of the sub-categories
are: Corporate, Real Estate, Banking, Employment, Bankruptcy, and several
items under each, which would look something like the following...

Business
Corporate
Mergers
Contracts
Real Estate
Purchase
Sale
Financing
Litigation
Employment
Labor Negotiations
Bargaining
Bankruptcy
Creditors
Debtors
Trustees

I want a required field where the user can select a main category (Business
or Litigation) and be presented with options to choose a relevant
sub-category then be presented with options to choose a relevant
sub-sub-category. The form contents will be emailed, so the results might
display Business - Real Estate - Sale. Thanks in advance.
