excel toolbars File Format Insert Tools aren't working

My Top toolbars in Excel, e.g. File, Format, Insert, Tools etc aren't working
and have also lost use of Cognos Analyst through Excel.

I would do two things. First, disable the Cognos add-in (I assume it
is an add-in -- in Excel go to the Tools menu, choose "Add-Ins" and
uncheck the item in the list). Then, restart Excel. If the problem is
fixed, then the Cognos add-in is the problem, so you should contanct
the vendor for an update. If the problem is still there, go to the
View menu, choose Toolbars, and ensure the correct toolbars and
checked in the list. For each toolbar that is checked, select it in
the list and click the "Reset" button in the dialog box.
