Tools, Trust Center, Add-ins, manage, COM Add-ins, Go, tic the box.

If it isn't listed you can "Add" and navigate to and select the
"cpaoaddin.dll" in C:\Program Files\Microsoft Office\Office12

I tried that. First, I found it in Program Files (x86) folder, I don't think
that makes a difference. When I select "cpaoaddin.dll", nothing happens. It
doesn't get added to the Add-Ins available list or anywhere.

Then we come back to this.

"Then I wonder if it isn't something to do with the security settings in
Vista? I'm not real familiar with Vista, but installing with Admin.
permissions and UAC disabled usually fixes most of the gotchas."

See if you can uninstall it in Add/remove programs, and then install as
referred to above.

Just as a follow up... I found out that it had to do with the CPAO Addin
Dlls. Here's how I was told to fix it and it worked.
1. Go to your program files -> Microsoft Office -> Office12 folder and
check if you have the CPAO dlls there. If there is a cpaoaddin.dll then do
step 2

2. Make sure Outlook and CPAO are not running, then

run a CMD window as administrator and type: regsvr32
"%ProgramFiles%\Microsoft Office\Office12\cpaoaddin.dll"

3. Restart Outlook and CPAO
