Duplicate headings appearing in TOC

Whenever I update the TOC in a particular document, it enters the "Table of
Contents" and "Introduction" headers twice. I end up manually deleting the
duplicates. I noticed that it doesn't happen in another document, so I tend
to think that it has to do with the original setup of the TOC (I did not
create the original document).

Any suggestions?

"Table of Contents" should not be included at all. Make sure that that
heading is in a style that is not included in the TOC (I usually define a
new style, based on Heading 1, called TOC Head). As for "Introduction," if
there is a manual page break immediately before the heading, which inherits
the Heading 1 style, you would get a blank entry before "Introduction," but
I wouldn't expect to get it repeated unless there is a TC field in the
heading.

I too am facing a similar problem.
In my document, few headings are being repeated in the TOC. Before the
heading, there is a manual page break, but in the TOC, there is no blank
entry before the main entry - the entry is just getting repeated twice.
I have another exactly same document where no duplication occurs.
Please help!!

Press the ¶ button on the Standard toolbar to display nonprinting
marks. You'll probably see at least one dot (·), which represents a
space character, or an arrow pointing to the right, which means a tab
character, immediately to the left of the page break. This can cause
the heading text to repeat in the table of contents. Delete the
characters and update the TOC.

Note that it is better to avoid manual page breaks and instead add
"Page break before" formatting as needed. You'll find this option on
the Line and Page Breaks tab of the Paragraph dialog box
(Format>Paragraph). If a particular heading should always start on a
new page, you should add "Page break before" to the style.

I was having a similar problem (duplicate TOC headings due to page breaks),
but after fixing the breaks, I found the problem returned when I added a
graphic to the otherwise blank page (preceding the page with the heading).
Though it pained me to do so, I managed to fix the TOC problem by adding
blank lines behind the graphic.

If you pressed Enter at the beginning of a heading to get a paragraph into
which to insert the graphic, then at worst you might have gotten the graphic
into the bookmark for the heading. At best, you have inserted the graphic in
(or anchored to) a paragraph in the heading style. The simple solution is to
change the style of the text paragraph in which the graphic sits (or to
which it is anchored) to Normal.
