disk defragmenter NTFS module has encountered a problem

I can't defrag my computer. It has been running really slow lately, so I decided to defrag it. When I click on analyze or defrag it says there is a problem and it says "the connection to the defrag engine has been lost. restart defrag". I try to restart it and it just continues to say there is an error.
also my disk clean up never works...it gets to 3 clicks on the time bar and then it stops and sits there for hours...I've left the computer over night and it still is at 3 clicks when I get up in the morning.
if anyone has any ideas on how to fix this, please help.
