Defragmenting always says "System Performance Can be Improved

Everytime I click to defragment in Vista, it always analyzes the disk and
says "system performance can be improved". Even right after it has
defragmented and I then analyze the disk again, it again says "system
performance can be improved". It is never ending.

Anyone have an idea how to fix? Thanks for the help!

There are more verbose options if you run defrag from the command line

Start > cmd > Right-click 'cmd.exe' ---> Run as admininstrator

defrag /?

Lack of available disk space might be one reason. Under XP you needed
approx. 20% free space on a drive for defrag to work properly. May well be
similar under Vista.
