Database Interface Wiz - Cant add records

I successfully created the database interface with the wizard. It reads all
the records existing in the database, good so far! I can delete records using
the "delete selected records" button, good so far!, now the problem If I
click the "add new record" in the Database Editor page, the lower half of the
screen displays "page cannot be displayed". Have I missed something please.

Sorry some additional info that may help Advisors to track the problem.
The "Submission Form" link in the "Database Editor Page" also returns a page
not found.
Q. do you need FP extentions for this function to work?
