Changing a table to a data entry form

I have a document that contains multiple tables that we use at our school for
our students' gradecards. It contains two tables, one for grades and one for
attendance. We print off one card for each student and then the teachers
handwrite in the grades for each subject and their attendance each nine weeks.
Is there any way to convert it to a form so that the teachers can enter the
students' grades right into the tables without my having to make sure that
they don't change the format of the card?
Any help would be greatly appreciated.
