can't get file upload to work!!

I'm at my wits end - I cant get file upload on a form to work. This is the
error message I get:
Cannot run the FrontPage Server Extensions' Smart HTML interpreter on this
non-HTML page:
"http://www.herlpitt.org/MEDIA_request_form_intranet/confirm.htm"

We already have FP extensions installed and working for other types of forms
on the site (forms that go to databases and e-mail addresses)

I have tried and read everything on the help and in these forums and nothing
works!!
Any suggestions?

Change the file extensions for your upload form to shtml

Did you follow

and does your host support file upload
