bad blocks

when i did a system analysis this message kept on repeating itself..

Disk The device, \Device\Harddisk0\D, has a bad block.

Is it a hardware or a software problem?
[and for the million dollar question....]
How do i fix it

Q1 - Hard ware problem.
Q2 - New hard drive.
