Access to Custom Contact Form for all users Outlook 2003

I have created a custom contact form in Outlook 2003 and published into
Personal Folders. I would like to allow access to the custom form for each
of the other 3 users on our server. We do not have Exchange and were told we
could not utilize Organizational Forms. How can everyone else access this
custom form so that we can get it up and running and utilized for the
company? How do I make this a Public Folder?
Any help is appreciated.

Each person will need to publish the form to their Personal Forms library.
