A Final Request For Medical Records Template for MS-Word 2003

You need to develop a template for A Final Request For Medical Records from
doctors, dentists, specialists , etc.

A lot of times these professional medical doctors, dentists and specialists
don't respond to us patients (consumers).

I think that this would really be a neat one to have to that this will knock
some common sense to these health providers.

Don't be silly! - What makes you think that having such a template available
from Word would have any influence on health care providers? If they wanted
such a thing they would do what most people do and create their own. If you
have an issue with the health care providers, take it up with them.
