A Disenrollment Request template for word

I found I need a workable template for creating a Disenrollment Request form
letter. I think these will be used quite a lot in the coming months, since
not that many people are familar with writting a request of this nature.

I have no idea what sort of Disenrollment Request you are referring to, but
