One is from All Users or Default User. Move or delete the shortcuts in one
of those Startup folders, then there should be no duplicate in your profile.

The whole Start Menu is a combination of these two folders...
C:\Documents and Settings\All Users\Start Menu
or %allusersprofile%\StartMenu
and
C:\Documents and Settings\Logged On User\Start Menu
or
%allusersprofile%\Start Menu

There should not be two Startup folders in the Start Menu.

I would suspect that one or the other %allusersprofile%\Start Menu or
%allusersprofile%\Start Menu is messed up.

Instead of doing all of the following, try Reset the Shell Folders paths to
defaults first. See below.

I can only guess that Start Menu and Startup are both messed up in both
keys. I do not know for sure. Maybe only Startup is screwed up.

Open Registry Editor and navigate to the locations below

Backup the registry before modifying it. See here.
http://windowsxp.mvps.org/registry.htm

HKEY_CURRENT_USER\Software\Microsoft\Windows\
CurrentVersion\Explorer\User Shell Folders

Double-click Start Menu and set it to %USERPROFILE%\Start Menu

Double-click Startup and set it to %USERPROFILE%\Start Menu\Startup

Navigate to this location:
HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\
CurrentVersion\Explorer\User Shell Folders

Set Common Start Menu to %ALLUSERSPROFILE%\Start Menu

Set Common Startup to %ALLUSERSPROFILE%\Start Menu\Startup

Close Registry Editor and restart.
