Zip Code Lookup

I'm trying to get my contact form to auto populate the
city and state when a zip code is entered. I've set up
relations between the data, but it's not bringing the
info in. Any ideas?

Does the data show properly when you run the query that the form is based
on?

If not, how did you import the ZIP codes into Access?
Sometimes it is possible to import leading or trailing spaces so check that.
I assume these are 5 digit ZIPs and not 9.
