Yes/No selection

I have a form that has 5 quesitons on it. Each of the five questions has 4
yes/no answers. I need help on how to make it so only one answer can be
selected at a time. Thanks in advance for all your help!!
 
You can do that with Optiongroups (hope I translate that correctly). One
Optiongroup for each question and instead of writing yes/no you write the
answer behind the checkbox.

I assume you mean you want the users to be able to select only one of the
four answers.

Add an Option Group to the form using the Control Wizard (on the Toolbox).
Follow the prompts to create your options.
