Wrong Reviewers Listed in Track Changes

When vieweing a Track Change document to "show reviewers"
a few of our users often see the name of a reviewer in
the list that never touched the document, someone who
doesn't even work for our company. The document was
created from scratch, so we know the name isn't there
from a previous version. The incorrect reviewer may have
been an actual reviewer in another document, but not the
one in question. To complicate matters, someone else can
open the document and "show reviewers" and the incorrect
reviewer doesn't show up in their list of reviewers. Has
anyone else had this problem and, if so, do you have any
idea why it occurs and how it can be resolved?

Change tracking is buggy, unreliable, and unsafe. I have
forbidden it to be used in our operations, because it
creates far more problems than it solves.

We review by sending a PDF file, and collecting e-mail
responses. If someone has extensive mods, I may send a
"docName_theirName" file, and incorporate their changes
after review. (This rarely happens.)

We have operations (and reviewers) on three continents and
in five time zones - with up to 15 hour time differences.
Change tracking created too much extra work (to fix
document problems), and too much ambiguity.

I think some add-in(s) put a bogus reviewer in the list of reviewers...
Yep, googling for "reviewer add-in" turned up ScanSofts TBRun97.dot
(TextBridge OCR software), which adds "Allan Stratton" as a reviewer.
