Why does Word load a file on startup?

When Word 2003 loads, i get a file that is loaded. It is an old one I copied
and pasted. I can't get rid of it using Clear Clipboard, setting Templates
to no template, and so on. This same document also loads when I open Outlook
on a reply to an email message.

On every Word doc and email reply, I have to highlite and delete to get rid
of the garbage. How can I get rid of this mess forever?
