Why does index repeat main entry?

I setup an index in a 200 page manual. It all looks good except for one
series of entries. For one word, the index keeps repeating the main entry
with just one subentry under each rather than accumulating all the subentries
under one main entry. As far as I can tell, the main entry is spelled the
same in all of the entries. Wiping out some of the entries and re-creating
them failed to correct the problem.

I can't offer a solution, but I can commiserate, as I had this exact thing
happen in a long document. I found it curious that the particular name I was
having trouble with was "Link" (which happened to be the main family name in
the family history/genealogy in question). I tried everything anybody
suggested, including (IIRC) treating the document as corrupt and cleansing
it. Nothing availed. In the end, I just had to wait till editing was
complete, generate the index one last time, and then unlink it and fix those
entries manually. Tedious but effective.
