Why does every message say "[SPAM]"

I just installed Outlook 2003. Every message subject starts out as "[SPAM]".
I have added all my contacts to the safe list. I have my junk email options
set to low (in fact, no mail messages are going to the junk mail folder). I
can't find any other option to turn this off. Problem is when I reply to a
message, all my recipients see the "spam" subject (unless i remember to
delete it).

This is not due to anything in Outlook - what addins do you have?

This is not added by Outlook. See if you are running an anti-spam add-in
that does this or contact your ISP to check if their anti-spam solution does
this.
