why does an e-mail address on my "safe list" appear as spam?

This has been annoying me for ages: every time I receive an e-mail from one
of my closest colleagues (working in a different location and for a different
company), it shows up with [SPAM] in the message's subject field. This
person is on my safe list, contact list, and sends me several messages each
week. Why is this when real spam seems to arrive safely and regularly in my
e-mail intray? How can I correct this for good?

It's really an annoyance because when I reply to her messages I need to
manually delete the [SPAM] from my subject header. Thanks for any light you
can shed on this.
 
that is done by antispam software running on your mail server or outside of
outlook. Outlook doesn't add the spam tags - it just moves the message to
the junk folder.
