- Click the Office button.
- Click Word Options at the bottom of the menu.
- Check the box for "Show Developer tab on the Ribbon".
- Click OK.
- On the Developer tab, in the Controls group, click the Legacy Tools
button (fourth one in the second row). The Forms tools are on the
dropdown.

I have just spent 2 hours tyring to make the check box work, now you are
telling me to go some place else which wastes even more of my time. Who is
wasting whose time, Mr Freedman? I still don't know how to make it work.
Unless you are the Mike M who posted this question 8 months ago, I wasn't
telling you to go anywhere else. Mr M posted his question in two places, and he
would have gotten a notification of Beth's answer.
The usual cause of a check box "not working" is that you haven't protected the
document for forms. That's on the Protect Document task pane, which is opened by
the Protect Document button on the Developer tab. If it's anything else, please
describe what "not working" means to you.
