Where is my appointment template (MikeSharp)

I note that if I look at tools/forms/choose form/appointment (in the
Standard Forms Libraray): I receive this notice: "Form Load Failed".

Is this why it takes forever for a blank appointment form to be created?

Whats the fix?
 
Version of outlook? have you deleted the frmcache yet?
