Where do I obtain a rebate log templete?

Looking for a rebate log templete to monitor coupon rebates sent to
manufacturers. Any help will be appreciated.
 
topdog, maybe put manufacture in column A, date rebate sent in column B,
date you received rebate in column C, if that want work post back with more
details as to what you want to do
