Where can I finda a template for the MACtac SR6017 label sheet?

Any help would be appreciated. i would like to print custom text on these
labels.
 
MACtac labels are listed under Other/Custom in Word's Envelopes and Labels
dialog, but all the stock numbers seem to begin with 62, 66, or 67. You
might find one that was similar, or you can use any instructions that came
with the labels (or a ruler) to create a custom label definition.
