What is the format for an actor's resume? (with three colums)

Does anyone know the correct format for a professional actors/actresses
resume? How can I download this form to word? Is there one most accepted
form? I've seen some with everything in three colums under separate headings
for theatre experience, training, education (etc.) but I don't remember
exactly how it looked. Thanks.
