It didn't come with a OS disk.
Can I create one and if so how?

Depends on your vendor and how it was set up. It's fairly common for
computers with preinstalled operating systems to come with a
restoration partition, and that partition can also be used to create a
DVD. Check in your manual or with your vendor to find out what you can
do.

Was it custom built?
Was it a big name unit (HP, Compaq, Acer, etc.)
Have read the manual that either came with it or is on the Hard Drive?

It is a Lenovo that was a replacement from an original. The warranty is
expired, so I can't get any answers from them.
We have moved since receiving and I have lost or misplaced all the

Hopefully, then, the Lenovo support web site would contain definitive
answers for you. Alternatively, you could seek out Internet support
forums used by other Lenovo customers who have experienced and resolved
this issue.

Contact Lenovo support at 1-800-426-7378 and request a replacement disc.

yup. worse case scenario you'll have to pay around $50 for recovery CDs.

If Vista came pre-installed and they didn't give you a disk, then most likely
there's a restore partition on the hard drive. That may or may not have a
"system disk" type folder in it.

If you're just looking for a way to guard against some sort of data loss in
the event that your HD crashes or something, I'd recommend creating a clone
of your HD using Norton Ghost, Acronis TrueImage, or equivalent software.
That way if you have some kind of drive error, you could get a replacement
drive and ghost that image onto it.
