Variables in a Form

I have a form in MS Word (Office 2007) where the user types in their answer
to certain fields. The form is locked (protection) so they cannot enter text
anywhere other than the areas that I want. Problem: Some of the fields
require the same text as others and I don't want the user to have to type the
same info multiple times. For example, the form might ask "What is your
doctor's name? Dr. ____" where the underscores show up as a fill-in field.
Later in the document, it says "Has Dr. (xxxxx) been made aware of your
condition?" I want the doctor's name area to populate with the name that the
user typed earlier.

Seems simple but cannot figure this out. Pls help!
