I have the same problem and the official response I got from USR was that
the 5416 WLAN card is "not supported in Windows Vista".
I have a USR "Max.G" 125Mbps network so I asked them which current, Max.G
compatible USR products would support Vista. They simply replied that "no USR
products support Vista".
USR's UK support aren't exactly a helpful bunch...!

Uninstall all the USR stuff first, follow the instructions on the above page
and install the TI driver. Use Vista's inbuilt Wireless Network Utility to
connect to your network.
And it works, because I'm using my 5416 on Vista RC2!
