User forms in PPT 2003

How do I create a user form in PPT and have the information from the
form go to designated points in the presentation?

Do you know VB or VBA?

Does your problem allow for a VB/VBA solution? That is, does it need to be
distributed using the free viewer or to users who may or may not have VBA
enabled?

If yes to both, then can you give concrete examples of what you mean by
"designated points"?
