User Feedback in FP2003

We have a form for user feedback to add to our web site that was made in
Word doc and exists also in a pdf. We can make the pdf for it can
"filled out" electronically.

Once we do that is there a way to click on a pdf and have it emailed to
an address?

I'm using FP2003

No. You can with a regular feedback form but not PDF.
