Use userform to select autofilter requirements

I'm trying to make my program look a bit fancier. At the moment I just have a worksheet full of data (such as customer, country, department) with autofilters. What I would like to have is a Userform with a combination of combo boxes and check boxes so that the user can pick the criteria there instead of on the worksheet.

Does anyone know a code for this? :confused:
