Use a hidden form field for storing users IP address

What do I need to use environmental variables as hidden form fields? I tried
using REMOTE_ADDR as the value and did not get anything returned.

There's a check box in the form properties to do that. Check under
"Options" and look around a bit. (It's not labeled IP address, but
something else.)
