URGENT: Search for text in column and count by month

Hi I am trying to find the cells containing the text defect and wann
count how many times it occurs a month.

Column G contains the defects text and Column S contains the month.

Eg:
Column G : Column S
Defect1 occured and resulted in failure : Jan-05
Defect1 and Defect2 occurred and resulted in failure : Feb-05
Defect1 and Defect2 occurred and resulted in failure : Feb-05

So my result should be:
Jan-05 Defect1 -> 1
Feb-05 Defect1 -> 1
Feb-05 Defect2 ->

Why wouldn't the results be:

Jan-05 Defect1 -> 1
Feb-05 Defect1 -> 2
Feb-05 Defect2 -> 2
