URGENT... Find Records Search Form

I want to create a form for a user to input whatever search criteria they
want into the blank fields and pull up the matching records from the
database. How do I create this? I'm on a time crunch and hope someone can
help me out. Thanks!!!
 
How many different field will you allow the user to filter on?
How much experience do you have in VBA?
