Underlining Mystery?

I "inherited" some HTML pages (see sample below) that use style sheets (see
sample below).

When I look at the code in Frontpage's design mode, most of the text is not
underlined (I don't want underlined). A few lines are. For example:
MEDICAL RECORDS (Chart Request)
PATIENT HEALTH INFORMATION

I don't want any text underlined and don't understand why this is.

More interesting (and confusing) is when I preview (either in Frontpage's
preview or the preview in browser function), all text in the division table
is underlined.

I am sure it is something simple, but I lack the knowledge. I appreciate
any assistance.

There are many mistakes/misuses/redundancies in your stylesheet.

Try replacing it with this -

To get rid of the underlines, all you REALLY need to style are the <a> tags.

The correct name sans-serif?
No quotes needed. "San Serif" is not a legitimate font class - it is sanS
Sans means "without".
