Unable to Publish form because of MAPI Error? HELP

I have created a form and trying to publish it; however, I receive the
following message

"Unable to successfully publish form ue to MAPI error. You must first
select icons for your form. To select iconc, click change Large Icon and
Change Smal Icon buttons."

I don't have any icon fields on this form. I also can seem to find the
Large and Small Icon button.

Will someone please help me get the form published?

YOu can set the icons on the (Properties) page of the form, in design mode.
