trouble after publishing web page

my page is at http://users.adelphia.net/~jh_rogers
after hitting my form submit button the info is to go to my email. it does
not and on my web page it is says [FrontPage Save Results Component] at the
top of the form. I do not know what is wrong here. Also I have tried to
publish my page with some new info in it (just text) and it says it is done
and published but it will not change on the internet. all help is valuable.

Does your web host have the FP extensions installed?
If yes, did you publish via FP's http mode?
