Transferring a custom form from one pc to another

I have created a custom calendar form on my home laptop, which has fields
for job number and workstage. I then use these fields to automatically
format the calendar.

As my company has now upgraded to Outlook 2003, I now want to use this
custom form. How do I transfer it accross from my laptop to my company
computer?

I use active synch and a Windows smartphone keep my laptop and company pc up
to date. Does active synch transfer these additional fields accross with the
calendar outlook items?

Any help, would be greatly appreciated.

Save the form as an .oft file, get it to the new computer, run the .oft file, then use Tools | Forms | Publish Form to publish it.

I don't think ActiveSync knows anything about Outlook custom fields.
