toolbar buttons added keep disappearing

I've added buttons to my toolbar several times and every time I go back into
Word, they are missing, what am I doing wrong?
 
Go to View/Toolbars/Customise/New to add a new toolbar of your own, call it
yourname.
Display your new toolbar form View/Toolbars.
Add whatever buttons you want.
Easy, it's always there, available from every doc when you need it.
