Use the Windows Explorer ("My Computer" or whatever your flavor of Windows calls the file manager program) to go to %appdata%\Microsoft\Word\Startup If there is a file there that looks like it belongs to the unwanted program, delete it. Restart Word. That usually works, but there are a couple of other kinds of add-ins. If necessary, see http://office.microsoft.com/en-us/assistance/HA011514521033.aspx.
