Text Form Fields

I'm designing a transmittal form that has the "To" information on the left (4 lines for name, address, etc.), and the "Date", "Trans #", "Project #", etc. on the right side of the top section

I have my Text Form Fields in place, but when typing the name and/or address on the left side (in the form field), the right side tabs over accordingly (before the empty space on the left is filled up), leaving the top of the form all out of whack and no longer lined up nicely

How can I keep the information and fields on the right side from moving?
